you can feel better in the comfort of your own home. We specialize in care and daily living assistance to an array of individuals. Whether you need daily or weekly assistance due to your disability, our care givers will provide an individualized service that you can trust.
Support Coordination is designed to support participants to make the most of their NDIS funds. Once a Plan has been approved, participants work with Support Coordinators to determine how their funds will be spent and to connect with providers.
Specialist Support Coordination
This service is provided by a professional with an Allied health background, Social worker or someone with Psychological expertise. This is service is mostly for complex participants who have exceptionally high needs or in a crisis situations.
With our Support workers we can help you to keep your home just the way you like it by assisting you with your household tasks.
They can help you with your shopping, whether its a trip together to the shops, or shop on your behalf if you prefer. Our professionals can assist you in preparing your meals and introduce you to new recipes or stick to your favorites if that's what you prefer!
Now you can get more involved with your community, be social, and participate in recreational activities with our range of social and community supports designed to help you enjoy your hobbies, see your friends, and have some fun.
If you are an NDIS participant and have been approved for the support category 'Core Supports’, you are able to access 'social and community participation' activities using your NDIS plan.
Many NDIS participants have different type of therapy in your plan, such as physiotherapy, speech therapy, occupational therapy or support from a dietician.
This service Includes short and long-term supports that focus on strengthening your ability and skills to coordinate your own supports independently, in order to develop your capacity for you to live independently and participate in your community.
Our individualised, person centred plans ensure the best quality support is provided to our clients to build their skills.
Group activities supports you to meet goals relating to participating in community, social or recreational activities. Such as funding for a Support Worker to support you to travel to and attend a community event.
This funding also covers support for activities that are either centre-based (e.g. to attend a Community Hub) or out in the community (e.g. to attend a book group).
We provide Early Childhood Support providing many therapies, and a key worker for the families with children 0-6 years old who has been diagnosed with developmental delay or disability.
We believe families know their child best and here at Priority care we have a family centered services that understand the parents and caregivers have the most powerful influence on their child's development. We partner with families to ensure that they become the main stake holders in what happens with planning and intervention.
You can get the best experience as a priority care participant with experienced suppport staff assisting you with supervising, personal tasks of daily life to develop your skills to live as autonomously as possible.
These supports are designed & provided individually to our participants needs. There is flexibility in ways the support can be provided in a range of environments.
We work with partners to provide workplace assistance which enables you to successfully obtain and/or retain employment in the open or supported labour market.
This service Includes:
employment related assessment and counselling
assistance in specialised, supported employment
employment preparation and support, individually or in a group.
We provide training and development activities undertaken by you or your carer to increase your ability to live as autonomously as possible.
Training can be provided either individually or through group activities and can include skills in daily life activities, communication and social skills, problem solving and managing funding of supports.
Assisting with and/or supervising tasks of daily life with a focus on developing the skills of you to live as autonomously as possible.
The support is provided to each person living in the arrangement in accordance with their need. This may include participant’s living in the same household or in cluster units or participant’s living in a host family arrangement. This activity occurs in a group living situation that can be either ongoing or temporary.
